Support coordination in the valley is a different job to support coordination in the suburbs, and any coordinator who tells you otherwise has not worked here. The constraint is not choosing between providers. It is finding providers who will actually come.
A plan can look well funded on paper and still sit unused for months because the services it names do not travel past Lilydale. Good coordination here starts with a realistic map of who genuinely services Healesville, Warburton, Belgrave and Monbulk — not who says they cover Melbourne.
That means being straight with you about trade-offs. Sometimes the choice is a provider who visits fortnightly rather than weekly, or a telehealth option that works well enough for a support that does not need hands on it. Those are real options, and they beat a beautifully written plan that nothing is delivering.
Coordination in the outer east also means knowing where the services actually are. Lilydale, Chirnside Park and Ringwood carry much of what the townships do not, so plans here often need transport built in to reach them, and appointments grouped into one trip rather than scattered across a week.
We coordinate with what you already have rather than replacing it. If you have a local GP, a mental-health clinician in Ringwood or a provider who has driven up the mountain for you for years, keeping that relationship is usually worth more than a tidier provider list.

Is support coordination done in person or by phone here?
Coordination is one of the supports that genuinely works remotely, so most of it is by phone or video, with in-person meetings when they matter — a planning meeting, a hospital discharge, or when you would simply rather sit down with someone.
What if my plan funds supports nobody local delivers?
We say so early rather than at reassessment. Then we look at the options: a provider who will travel, a remote alternative, or evidence for your next plan that the funded support is not available in the way it was written.
