The south-east has no shortage of providers, which sounds like good news and creates its own problem. When a plan could be filled twenty different ways, the job of coordination shifts from finding anyone to choosing well — and knowing which of those twenty actually deliver what they advertise.
That is local knowledge, and it is why being based here matters. Our office is in Cranbourne and most of our team lives across Casey, Cardinia and Greater Dandenong, so the picture we have of local services comes from working alongside them rather than from a directory.
Coordination should also make itself unnecessary over time. The aim is that you understand your plan well enough to drive it — what is funded, what it is for, and what to do when something is not working.
We coordinate with the health services people here are already connected to: Casey Hospital in Berwick, Dandenong Hospital, Monash Medical Centre at Clayton, community mental health teams and headspace along the corridor. Discharge is a common moment for a plan to fall over, and it is a good moment to have a coordinator.
Choice matters more than convenience. We will coordinate you to another provider whenever they are the better fit, and our support coordination keeps clear separation from our other services so that recommendation is genuinely open.

Can you help after a hospital admission?
Yes, and it is worth calling early. Discharge is when supports most often need rearranging quickly, and we are used to coordinating with the local hospitals and community teams.
